*    INFLUENCE OF EJECTION FRACTION ON CARDIOVASCULAR OUTCOMES IN A BROAD SPECTRUM OF HEART FAILURE PATIENTS, by Solomon et al.
 
The degree of left ventricular (LV) dysfunction in patients with cardiovascular disease is an important determinant of prognosis. However, data supporting this association have predominantly been developed from populations with systolic dysfunction below a certain ejection fraction (EF) cutpoint, and for the most part from populations not studied with contemporary heart failure therapy. In this issue of Circulation, Solomon and colleagues examine the large Candesartan in Heart Failure Reduction in Mortality (CHARM) program database to assess the relation of LV function to outcomes across a broad range of EF (including heart failure patients with preserved as well as decreased LV function). The hazard ratio for all-cause mortality increased by 39% for every 10% reduction in EF below 45%. Risks of many unfavorable outcomes declined with increasing EF until an EF of 45%, after which the risks plateaued with continuing increase in EF within the normal range. These important contemporary data suggest that EF remains a powerful outcome predictor in the current era but when above 45% does not incrementally contribute to risk assessment. See p 3738.

Interventional cardiac procedures are critically dependent on understanding the relation between catheter position and cardiac anatomy. Electrophysiologists have become skilled at inferring the location of catheters on the basis of the fluoroscopic image and electrogram appearance without visualization of the cardiac anatomy.
