*    SECULAR TRENDS IN DEATHS FROM CARDIOVASCULAR DISEASES: A 25-YEAR COMMUNITY STUDY, by Gerber et al.
 
Age-adjusted cardiovascular disease mortality has dropped over the last few decades, but questions remain about whether there are differences in the rate of decline by the site of death (in-hospital or out-of-hospital) and by age and sex. The population of Olmsted County, Minn—the home of the Mayo Clinic—has provided investigators an opportunity to track these changes over 25 years, from 1979 through 2003, in a group with a fairly uniform medical care system. The study provides important public health information about the progress that is being made in the prevention of cardiovascular death and highlights where that progress may be lagging. See p 2285.


*    PREDICTIVE FACTORS OF RESTENOSIS AFTER CORONARY IMPLANTATION OF SIROLIMUS- OR PACLITAXEL-ELUTING STENTS, by Kastrati et al.
 
Although clinical trials have established that drug-eluting stents improve clinical outcomes after percutaneous coronary intervention, it is valuable to examine real-world experience with new devices after approval, in this case to define the optimal utility of drug-eluting stents in broader patient and lesion subsets. In this issue of Circulation, Kastrati et al identified predictive factors for angiographic and clinical restenosis after the implantation of drug-eluting stents in 1845 patients with 2093 lesions. Multivariable analysis showed that vessel size, final diameter stenosis, and drug-eluting stent type (lower with Cypher compared to TAXUS) were the strongest predictors of restenosis. These findings may prove useful in improving clinical outcomes after implantation of drug-eluting stents. See p 2293.
