Extraction of an Accufix Atrial J-Shaped Support Wire

A voluntary recall of the Teletronics Accufix models 330-801 and 329-701 J-shaped atrial pacemaker leads was issued November 3, 1994, as a result of several reports of fracture and extrusion of the J-shaped retention wire from the insulating sheath in these models.

A 70-year-old woman presented with a 7-year-old Teletronics Accufix 330-801 atrial lead that demonstrated intermittent lead failure. Fluoroscopy revealed that the J-shaped support wire of the lead had fractured and migrated from the insulating sheath (panel A). Since this type of wire (panel B) has been reported to cause laceration of the atrium and vascular structures, it had to be removed. The extruded retention wire was successfully snared with a 15-mm Micro-Snare (panel C) and removed percutaneously through a femoral SRO Daig sheath.
