The degree of left ventricular (LV) dysfunction in patients with cardiovascular disease is an important determinant of prognosis. However, data supporting this association have predominantly been developed from populations with systolic dysfunction below a certain ejection fraction (EF) cutpoint, and for the most part from populations not studied with contemporary heart failure therapy. In this issue of Circulation, Solomon and colleagues examine the large Candesartan in Heart Failure Reduction in Mortality (CHARM) program database to assess the relation of LV function to outcomes across a broad range of EF (including heart failure patients with preserved as well as decreased LV function). The hazard ratio for all-cause mortality increased by 39% for every 10% reduction in EF below 45%. Risks of many unfavorable outcomes declined with increasing EF until an EF of 45%, after which the risks plateaued with continuing increase in EF within the normal range. These important contemporary data suggest that EF remains a powerful outcome predictor in the current era but when above 45% does not incrementally contribute to risk assessment. See p 3738.

Interventional cardiac procedures are critically dependent on understanding the relation between catheter position and cardiac anatomy. Electrophysiologists have become skilled at inferring the location of catheters on the basis of the fluoroscopic image and electrogram appearance without visualization of the cardiac anatomy. The limitations of this approach are well recognized and likely contribute to procedural failures. Nonfluoroscopic navigation systems are available but expensive. Ector and coworkers have developed software for integrating magnetic resonance images of the right atrium into standard biplane fluoroscopy systems. Detailed anatomic structure can be related to electrophysiological findings. This technology has the potential to bring the relation of structure to pathophysiology into better focus with widely available fluoroscopic systems. See p 3769.

A heart-healthy diet introduced in early childhood improves a child’s lipid profile, but its effect on vascular health is unknown. To investigate whether early introduction of a low–saturated fat diet has a beneficial effect on endothelial function, Raitakari and colleagues measured flow-mediated vasodilation in 11-year-old children participating in a prospective dietary intervention trial beginning in infancy. Boys in the intervention group had better endothelial function than those in the control group (P=0.0034), but no difference was seen in girls. This effect was partially explained by diet-induced reduction in serum cholesterol concentration. Low–saturated fat diet maintained in the first decade of life is associated with enhanced endothelial function in boys. See p 3786.
