A 9-year-old girl had hypertension in her upper arms, bilateral
delay in her femoral pulse, and an abdominal bruit on physical
examination. Angiography of the abdominal aorta in the posteroanterior
view demonstrated saccular dilatations (aneurysms) and narrowing
(coarctation) at the level of lumbar vertebrae 1 and 2. There
is osteal narrowing with proximal stenosis of the left renal
artery and total occlusion of the right renal artery. A gradient
of 30 mm Hg was measured across the narrowed segment of the
abdominal aorta.
